COVID-19 PANDEMIC

IN RESPONSE TO THE ONGOING COVID-19 PANDEMIC THIS PRACTICE IS NOW LIMITING ALL FACE TO FACE CONTACTS WITH PATIENTS. ALL ROUTINE APPOINTMENTS ARE NOW SUSPENDED UNTIL FURTHER NOTICE FROM THE HEALTH BOARD. IF YOU NEED TO SEE A GP IN AN EMERGENCY PLEASE TELEPHONE THE SURGERY ON 028 90776600 AND LEAVE A DESCRIPTION OF YOUR SYMPTOMS. WHILST WE ARE EXTREMELY BUSY A GP WILL CALL YOU BACK. IF YOU WISH TO ORDER A PRESCRIPTION PLEASE DO THIS BY TELEPHONING THE SURGERY AND REQUESTING OPTION 1 FOR THE 24 HOUR REPEAT PRESCRIBING LINE OR YOU CAN NOW ORDER ONLINE. YOU NEEED TO REGISTER FOR THIS SO PLEASE CONTACT RECEPTION FOR DETAILS. PLEASE NOTE THAT ALL PATIENT NOW ARE OBLIGED TO NOMINATE A PHARMACY TO PICK UP THEIR PRESCRIPTIONS FROM THE SURGERY. WE NO LONGER HAND OUT ROUTINE PRESCRIPTIONS TO PATIENTS. THANK YOU FOR YOUR CO-OPERATION.

New Patient Registration

Patients can register with the Practice upon production of a valid NI medical card or by filling in the appropriate forms. You must live within the practice boundary area of 4.5 miles

Please contact reception to arrange collection of new patient registration forms. You will also need to provide some form of photo ID and a utility bill with your current address

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Changes to Personal Details

If you change your name, address or your telephone number please let the surgery have your new details.
